Consent & Registration

An informed consent form and a registration form will be obtained ahead of the first therapy session. These forms will require personal information such as demographics, need for therapy and will state information on confidentiality norms and payment policies. This practice is to ensure that the clients are informed ahead in time of what can be expected over our policies.

Ensuring Privacy

We strongly value work ethics in securing client confidentiality and privacy. The following are practices upheld from the end of the therapist to follow by our confidentiality policies.

Without the consent of the client, identity or information shared with the therapist during the sessions will not be shared with anyone else, expect for crisis based access.

Observations and progress during the sessions will be documented as therapy notes for the reference of the therapist. Information stored will be held confidential and will not be in access to anyone other than the client and the therapist.

Messages, mails and calls between the therapist and the client will not be recorded or shared from the end of the therapist.

To provide client privacy, if the therapist happens to see the client in public space, will not greet or initiate conversation from their end. The therapist will also not comment or interact with client’s personal social media unless the client reaches out. By norm, the therapist will not meet or take an invite from the client to meet in an informal setting away from therapy sessions.

Informed consent and confidentiality forms are gained from the client before the first session, while they sign up for sessions.

Ensuring Safety

If we believe you may benefit or require further medical care, a referral to a mental health firm / psychiatrist / clinical psychologist / shall be made to ensure the client gains the best care as needed. Upon request, we can also provide a referral letter with client’s history and the therapy treatment plan and progress they have had during therapy sessions, addressed to the mental health practitioner.
